If your child is transferring over to secondary school in September we need to know if they'd like to continue with their music lessons with us to enable us to confirm availability at their chosen school.

• Childs name 
• Secondary school from September  
• Continuing lessons Y/N

Please refer to our notice period in the application pack should you wish to stop lessons at the end of Summer term.
